Home Health

Essex County Health Department operates a certified Home Health Agency.

Bringing Independence…a Little Closer to Home since 1969!

We know home is the best place to heal. This is why we deliver medical care that was once only available in the hospital to our clients’ front doors. Home health care is a wide range of health care services that can be given in your home for an illness or injury. Our goal is to prevent hospitalizations, ER visits and allow patients to stay in their homes. We work with you, your family and your doctors to provide a hands-on care plan and personal assistance. We can link you to other community services and caregiver support.

Our Home Health Agency nurses provide home care specific to each individual’s needs. Services may include:

5 STAR Patient Satisfaction!

Home Aid Care ⬆

Telehealth home care lets your health care provider care for you without an in-person office visit. Telehealth is done primarily online with internet access on your computer, tablet, or smartphone. A telehealth coordinator will come to your home to show you how to use it. The tablet features large font and one click navigation.
The tablet uses digital medical devices, such as weight scales, blood pressure monitors, pulse oximeters, and blood glucose meters.
