We know home is the best place to heal. This is why we deliver medical care that was once only available in the hospital to our client’s front doors. Home health care is a wide range of health care services that can be given in your home for an illness or injury. Our goal is to prevent hospitalizations, ER visits and allow patients to stay in their homes. We work with you, your family and your doctors to provide a hands-on care plan and personal assistance. We can link you to other community services and caregiver support.

Nursing care is the foundation of our Home Care program. Our team of experienced, licensed Home Care Nurses delivers hospital-level medical care to you in the comfort and safety of your home. We offer a range of services to help you achieve optimal health and wellness and maintain your independence.

We provide physical, occupational and speech therapy care in the safety and comfort of home. Our team of experienced, licensed clinicians will work with you to heal after a fall, surgery, injury, or other incident. We are here to help you regain your strength and mobility and maintain your independence.

Telehealth home care lets your health care provider care for you without an in-person office visit. Telehealth is done primarily online with internet access on your computer, tablet, or smartphone. A telehealth coordinator will come to your home to show you how to use it. The tablet features large font and one click navigation.
The tablet uses digital medical devices, such as weight scales, blood pressure monitors, pulse oximeters, and blood glucose meters.
Receiving Services:
Services are available to any resident in Essex County.  All patients need a physician to establish a plan of care, sign orders for treatments and services, and provide oversight for patient home care. The types and duration of home care services depend on the patient’s need and their provider’s care plan.  We can receive referrals from physician offices, hospitals, nursing homes, rehabilitation centers, etc.
Paying for Services.
We bill your insurance company!
Patients are asked to provide insurance information.
If the patient does not have insurance, she or he will be billed on a sliding fee schedule based on income.
Payment for home care services are accepted from various sources:
Aetna, Blue Cross/Blue Shield, CDPHP, Excellus, Fidelis Care, Medicare, Medicaid, NYS Empire, United Healthcare, Veterans, Worker’s Compensation
