Registration & Consent
These tests use either a saliva/inside the mouth swab or a nasal/inside the nose swab.
For each test, your student will swab her/him/them self or, if assistance is needed, by the school nurse.
Once samples have been collected for the day, they will be sent to Quadrant’s lab to be tested. Results will be ready within 24-48 hours and can be viewed by logging back into app.Clarifi-COVID-19.com.

SYMPTOMATIC TESTING FOR SICK STUDENTS with the Abbott ID NOW
The Abbott ID NOW COVID-19 Test is a very easy nasal swab and results are available in 15 minutes. This test can be used by our school health office staff to rule out COVID-19 if your student becomes sick during the school day.
A negative test result does necessarily allow your student return to class. Your student must still follow the district’s sick/return to school policy. Your student may still need to be seen by their healthcare provider. A follow-up COVID-19 test might be recommended.
For your student(s) to participate in symptomatic testing, you must:
- Complete the consent form below &
- Check the Symptomatic Testing option.

EXPOSURE-BASED SCREENING TESTING with the Abbott Binax Now
The Abbott Binax Now test is also a very easy nasal swab and results are available in 15 minutes. This screening testing option is for vaccinated and unvaccinated students who were exposed to a positive case during high-risk school-based activities. These activities can include close contact or indoor sports, band, choir, theater, and indoor school clubs.
For your student(s) to participate in exposure testing, you must:

- Check the Exposure-Based Screening Testing option.

Results of all tests must be reported to the local health department following New York State Department of Health Communicable Disease Reporting Requirements.
